Split it between small, mid, and large cap index funds
The S&P 500 index funds are typically large cap. Then when you hit 50 (or so) start transitioning those holdings to income and/or bond funds.
Most investment companies will encourage you to roll over your 401k into an IRA which allows you to pick from any fund or stock you wish.
