Insurance companies resulted because of high medical costs
And at the demand of the people. So according to the logic of the OP, providers and the public would be too blame. Providers get way too little blame in all of this.

I would also separate for profit from not for profit insurance companies as those are vastly different things traditionally.

However with medical loss ratio in place I am not sure anyone can claim insurance companies are making windfall profits
