#3 is misleading. There is usually contact when someone reaches over another

player to get a rebound. In fact, I can't remember the last time I saw someone reach over someone's back without contacting that player. If someone is boxing out (i.e. contacting another player with his/her back), and a player reaches over that player to try for a rebound, that is a foul. It happens way more often than it's called, IMO.
