Haven't been there for years, but...
The best shake when I lived there was at ...honestly... the snack bar at the Utah County Hospital. Ice cream and milk, drinkable with just a hint of solid ice cream remaining. Perfection.
I'm going to have to check it out next time I drive to Provo.
