My wife had both our kids with no epidural but she had morphine given for pain

management. She got spooked but a couple of people she knows who had bad experiences with epidurals. She did just fine and the pain was minimal. The hospital we went to had a great maternity area and the doctors were great. Our regular doctor did not arrive in time for our second child but the resident who delivered was amazing and I wish I would have caught her to thank her before she disappeared once our regular doctor arrived.
