This does happen from time to time, and it's awful, but what really seemed weird to me was that the girl was going in for two cavities and the dentist went in to the waiting room in the middle of the procedure to let the mom know she needed four crowns as well.
I don't do a lot of pediatric dentistry, but needed a crown at that age seemed really odd to me, as those teeth had only recently erupted. What made me truly sick was that the medical examiner doing the autopsy found nothing wrong with the teeth that were being worked on. According to the ME, the work was unnecessary and her death was completely pointless.
So sad for that little girl and her parents. I can't even imagine.
