Talk to your Dentist face to face about it. Most hope/assume you won't look at
the charges. Ask him to waive it.
If he doesn't......cancel the appointment.
Plenty of Dentists in this world. Do you want to work with one who obviously has no problem nickle-and-diming you to death?
