Go to Austria. Once the guy is being treated, call the US Embassy in Vienna
The ACS section can help. I work at an Embassy (Malta), and our ACS unit helps people like this all the time. We just had an AMCIT break his leg on a cruise excursion and the ACS chief went to the hospital to visit the guy, and helped him make plans to get better and then return to the US. The ACS unit in Bosnia should have also been able to help, but those are small Embassies in the Balkans so who knows what their staffing, etc... is like.
