Well, there are a few things, mostly guesses
I think the fact that he is having to adjust the offense so drastically is indication that preseason evaluations of how the offense is performing is off.
His QBs aren't making good reads in-game. He is the QB coach so I would hope he knows that they are struggling before game time.
He's learning, I have hope he'll get it together, but you can't go this long with this kind of offensive performance blame-free.
