Do you have documentation that they made a mistake?
In other words, did you fill out a form showing that you chose the traditional PPO plan but they mistakenly enrolled you in the wrong plan. If you have document proof, the broker should be able to correct that with the plan carrier and retroactively pay your eligible claims.

So you need to talk to your HR department and find your completed enrollment form and then work through them with the broker for your company. Not a big deal to correct it if you have proof.
