inside and outside digs are known before the ball is snapped
The QB is releasing the ball on a pass that far, before the WR even starts his plant for the comeback, that is not an on the fly decision by the WR or QB.

As the WR is running that route, he knows exactly on what yard line, and with what foot he is going to plant depending on in or out after the comeback or that play would rarely work.
Because they always start from the LOS with the inside foot as the forward foot, it changes the route significantly as to whether the route design is inside or outside, so it is 4 different timing patterns for the QB to throw depending on which side of the field and in/out for the exact same yardage. That isn't something you can just wing it on during a game and hope it works, those routes are very complicated.
