We were an exception, but my wife got pregnant while on an IUD.

Most of the time a pregnancy occurs with an IUD it was because the IUD, er, um..."wiggles out" so to speak. Dr confirmed that was not the case for us. There may be experts here on the board that would be more knowledgeable, but that's my anecdotal experience.
