Usually several factors added together make for disaster
Like the Titanic, one or two factors were overcomable. Three or four were not.
A pilot friend told me the three cardinal sins were:
1. Flying in bad weather
2. Feeling like you HAD to fly in order to get somewhere.
3. Overestimating your ability to handle all situations.
Seems like the pilot may have violated all three
