Inaccurate numbers during pandemics is why the Spanish Flu has its name
At the time, during WWI, Spain was a neutral country and was not motivated to try to convey strength by minimizing the number of sick people. Other countries were so Spain looked like they were the hardest hit. The disease came from a region of France, not Spain, but some have theorized that it also came from, you guessed it, China.
