Systems being overwhelmed and running out of resources. This is literally happening now in NY, Louisiana. CV disease, other acute emergenices doesn’t care about covid 19 and will still happen at a steady rate - hard to properly care for these patients when your system is completely overwhelmed.

Plus those numbers were based on estimates - go look at actual CDC website to see the final tally. Wasn’t quite that high.
