Do it directly from the state and not through a service - saves money.
Once you get your LLC "Articles of Incorporation" (they come with your registration) go to IRS.gov and get an EIN number using your LLC information. Then you'll be able to get a bank account with your LLC name. Keep your money from the business separate from you personal money.
On your taxes do a Schedule C for your LLC and you'll be good to go. Quite easy actually.
