My company is in the dark ages with regard to time off. One week paid vacation. Use it for whatever you want. Sick, vacation, bereavement, paternity/maternity, etc.
In addition, 4 paid holidays. New Years Day, 4th of July, Thanksgiving, Christmas.
Other than those days, if you're an hourly employee and you're not there, you don't get paid. If you're salaried, you basically just don't abuse it and there's no problem with you coming and going as you want. I could take a few weeks/year and they won't really care, as long as I get my stuff done.
