Slim Intervention Needed
No one knows what it actually means. Additionally there are lots of documents drawn up by lawyers RE end of life care
that were clearly drafted by someone with no practical experience in the medical field. Unfortunately people create these documents with the help of the lawyer and then think they have their end of life wishes in order and the documents end up being useless.

The documents will say something like “in the event that two physicians deem I will not make a successful recovery blah blah blah”

That type of wording is almost never useful.

By the time that type of document becomes useful you’ve usually put the person on some type of life support that should never have been started in the first place. Of course there are exceptions but those are rare.
