Insurance was Cigna, and our plan was contracted with IHC hospitals. We didn't find out that the ER physician didn't accept Cigna until we got the bill for several hundred dollars from his physician group, and we called them up, and we're told we were out of network. Turned into a battle of over a year with Cigna saying we owed nothing other than the ER copay and the physicians office telling us to pay up or you're going to collections. Thankfully they gave up in the end and we didn't pay them anything extra, danged thieves.
