Tip based on number of items ordered rather than the price. If you have a large
order that includes a whole bunch of apps, drinks, etc, then you tip more than if you just ordered entrees and nothing else.
For example, you could do $X per entree ordered, $Y per appetizer, and $Z per drink regardless of the price of those items.
