Ok, let me do your research for you.
A: It took me about 30 seconds to find it. This link is to a screen grab of Brad Van Vark, Big XII official who was the lead official in the BYU/Utah 2016 game. Email the BYU SID office if you don't believe me.
B: It's been my experience that most games that teams lose were "poorly officiated".
C: Refer to A. The BYU SID office will confirm this. If PAC-12 officials have ever been used at Rice-Eccles it was because BYU declined to supply officials. If you look at the contracts for BYU/Utah games - which are easily found online as they are public record due to Utah being a non-private school - you will see in all of them that the visiting team has the option of supplying officials.
