It's easy to get the costs. Ask for the cpt codes and call the facility where
The procedure will be done. Get quotes from the hospital and the anesthesia dept. every hospital has someone that does this.
Ask the surgeon how much they charge.
Then call your insurance and figure out with them what is considered allowable. Subtract the percentage that your insurance will pay and then figure your own costs.
