Gary Crowton, hands down, 2nd place isn't close, he was HC but he ran the 'O'
He never settled on an offense. I think he read too many news articles that he was an offensive genius. BYU offensive teams under him never looked like they knew what they were doing and he didn't use the personel he had effectively. When Brandon Doman graduated (after being successful as an option type qb) Crowton tried using Matt Berry and Brett Engemann, who my grandmother could have out run, as option qb's. I don't know that either of them ever gained a yard running an option play.
