My employer pays all of the premiums. This is for a HDHP with a family
deductible of ~$3400, and an annual out-of-pocket max of $5100. They also throw in like $250 to your HSA if you jump through some health checkup hoops every year.
They offer some other more traditional plans (like PPO and HMO), but the premiums are higher than my worst-case out-of-pocket max.
