Miller is actually very good at keeping it suggestive rather than explicit -- the worst is all quickly cut or takes place off-screen. It's definitely R, but there are plenty of war movies with more gore. The bad guys are something of a freakshow, but not to an unpalatable degree. Somewhat Lynchian -- the main baddie is similar to Baron Harkonen from Dune -- but dedinitely less than full bore Lynch.
