they any more or less reliable than diesel alone? Do they affect the power/torque positively or negatively? What about mileage - how positively do they impact it?

I've seen sites talking about how big rigs have used this for quite a while now, but their use in personal vehicles seems pretty new. It seems to be nothing but positive. Are the sites promoting their use simply glossing over their problems?
