They went in to clean that up and saw that the graft from earlier this year was loose enough that my knee would slip. That and part of it got into the joint as it bent per the surgeon.

This one is the fault of the piece and the manufacturer. My surgeon said he's never seen that before. The thing snapped in two right down the middle.
