I'm guessing that the pallet was initially much farther back in the truck
They had to use the pallet jack to pull it more closer to the door, and then knew they needed to pull the pallet jack back out so that the forklift could go in.
So in their attempt to pull the pallet jack back out, they ended up pulling the whole pallet with them to the point that when they tried to abruptly stop before the jack fell off the platform, the pallet rocked enough to fall forward and tip over.
