There is enough data for insurance companies to know exactly
what the risks are for any age, race, and sex group anywhere in America. They also have enough data to know the actual costs for every single procedure.

If there is going to be a change to the way we insure health in America, it should be to require Health insurance companies to group every single holder of their policies under one policy.
Risk is spread out across the board. They don't get to make sweet heart deals for x company and then screw over y company.

Competition then becomes how good is that health insurer and measuring the risk, managing their costs and marketing themselves.
