My name, email address, social security number, phone number, and street address were all leaked by AT+T. I don't have an active account with them, but they were my ISP at a prior address.
Only thing I would add to ID monitoring, credit freeze at all 3 agencies, fraud alert, and turning on multi-factor authentication for all important accounts, is if your mobile phone number leaked, you can have your provider put it on a "number lock" to prevent it from being transferred to another carrier.
