We are on a family plan with T-Mobile. The total bill is divided 10 ways. We each get 3gb of High Speed Data, unlimited lower-speed data, and unlimited everything else. It works out to $35 for our two phones, which is flippin sweet. That price does not include hardware. Nexus and Moto are your best options. For transparency, I paid $200 for a Nexus 4, 3 years ago and $350 for a Nexus 5 2 years ago. The N4 is at the end of its desirable life. I will upgrade it to a Moto X 3rd Gen as soon as they are available (Sept 3) for $400. Honestly, these phones rock. Even adding in my hardware costs, our total Cell-Phone expenses seem like a great deal.
Internet is with CenturyLink. I don't know the speed - we can stream Netflix, YouTube, etc. just fine. How much faster do you want to pay for?
Just signed up for a two-year fixed price with Dish. The top 200 package gets me all the ESPNs, Fox Sports 1, NBC Sports, CBS Sports, Big Ten, Pac 12. . . and some others? I asked for and receive an OTA adapter for free, so I will get BYU-TV in HD, integrated with my program guide and DVR (I live in UTAH).
So, yeah. I think we do it pretty efficiently and get everything we want.
