Generally I take them in if
1. They aren't staying hydrated
2. They can't breath (like croup I can't fix at home)
3. They are extremely lethargic (like I almost can't wake them up).
If it is JUST a fever, you can probably just watch at home and give tylenol if he's uncomfortable.
