In my community, NPs significantly over-refer even the simplest issues
and create a huge backlog of patients that significantly extend waiting times to see providers. And meanwhile, the patients who need specialist help on nan urgent basis have to wait longer. Not ideal.
Pn the other hand, I do actually think midlevels with proper support/supervision (not complete independent practice) have value, especially in underserved areas.
