Not exactly, but we do try to give warnings unless we feel the the person really should have known better due to previous warnings or just because it was really obvious.
Sorry, but #2 is a non-starter. There are very good reasons for that rule. We really don't give penalties (or even warnings) if you at least categorize it in one of the restricted categories. If you categorize it as a non-restricted category when it obviously belongs in a restricted category, then we'll give either a warning or a 1-hour penalty -- that's the minimum penalty possible.
I've considered #3, and I think we need to do a better job of having some rotation of them, but there are downsides to it as well -- see crazY's post for my biggest concern. It usually takes the better part of a year for a mod to get a decent feel for how we try to moderate.
