What I don't understand is why one type hasn't simply died out
I realize the 5th wheel is designed for even more weight than the gooseneck, but why not just do away with the gooseneck style? It can't be anymore expensive to make a 5th wheel trailer than a gooseneck trailer, speaking only about the hitch connection. The 5th wheel receiver might be more expensive than the gooseneck, but both require bolting to the frame of the truck, so even that seems like it would end up a wash.

Any ideas on why the gooseneck persists?
