They are pretty clear when something won't be two days, in my experience.
And while two day shipping is definitely one of the main draws to getting a Prime membership, Amazon doesn't guarantee anywhere that everything (even everything Prime eligible) will be delivered in two days.
I've only had one error in my four years of doing Prime. I have only had maybe two or three items take more than two days (and it was always made clear when I ordered those items that it would take more than 2 days).
