Update. My wife called the Costco Store Manager.
In typical and excellent Costco fashion, he told my wife that while he couldn't respond to the diagnosis disagreement, he agreed that the way the optometrist treated my wife "is not how they do business at Costco". $100 refund attached to her account.

Excellent choice. Costco does have great customer service.

What sucks for me, is now I have to pay twice as much to get an exam somewhere else...I don't have much confidence in the guy, but I will buy my glasses there, annoying whatever doctor I do use because they can't make the money on it...
