#3 might not be a bad thing. "For how we try to moderate" creates a sub-culture
in it's own.

It's not the profanity, nudity, innuendo, etc. that everyone knows (and most agree) should be moderated. It's not even deleting a post or two and throwing a poster in the slammer for a day (or a week) for trolling/baiting that we have an issue with. (In fact, my favorite penalty was being first to comment on the ref's post about "Anaologies regarding B12 invites are considered trolling and will penalized"....I simply posted "The ref has thrown the flag", got tossed in the slammer for a day (rightfully so), but I felt satisfied about it, and hopefully somebody chuckled.

Anywho.....the moderator "issues" as I see it, come from an unwritten mod-culture:

Issuing punishments or sentiments on "percieved intent", bad blood between certain mods and certain posters (everyone knows relationships, even with fake-internet friends/enemies is a real thing), etc. I understand certain mods may despise certain posters because of all the trouble and headaches they've given them, but then that bias is rendered as the sword of justice and the poster then retaliates by hating the mod even more, and the cycle gets worse.

This could be easily averted by some form of mod-roating schedule. Even if it was every two years or so, a "refresh" would get your mod-team back-to-the-basics. Mods could take a break and come back for another cycle if you perceive that a particular mod is rather good at his job. But still give him a break to "reboot" and see life as a common-folk again. . It's a good idea, and one you should probably try to implement somewhat.
