I think there were a few mistakes...and just some reality.
Mistakes:

1.) Not having a better qb to take over after Max Hall graduated.

2.) Anae leaving.

3.) Hiring Brandon Doman and his posse of young coaches.

Reality:

1.) Jake Heaps just did not develop and wasn't a very good qb. I don't know why or how or who, but he wasn't very good as a sophomore. He also sucked at Kansas so the data is pretty clear, he just wasn't good. Taking that reality into consideration really hammers home how bad the mistakes of #1 really was.

In totality and now that the dust has settled I believe it worked out the best for BYU in some ways. I think losing Anae rejuvenated him, it led to some poor coaches leaving BYU and I think has changed some attitudes. I think if Anae stays Jake doesn't suck but he is never as good as folks thought he was after his freshman year. Anae would have been run off by our stupid fanbase because he is the one to blame for the reality that Jake just wasn't very good. In fact, I wonder if the good Dr recognized that Heaps had more issues than others and might have left recognizing the train wreck ahead when ridiculous expectations of the most ill informed fanbase collided with reality. It was a sucky couple of years but last year had some light and hope and this year should be really, really fun. Even JimmyMac is getting in on the deal!

I think too many make too much of Jake Heaps transfer. He just wasn't that good and I think it likely went much beyond the CB from Mississippi taking his head off. I think that my conclusion that Anae saw the train wreck coming is likely one of the most lucid and well thought out comments on the subject, but that is pretty much why I keep my humble self here. Stick around I will be back again sometime.
