"R" almost always is "R" because the content is fairly objectionable, and something we probably don't need to see. "R" isn't the standard (many PG-13's are raunchy), but it's pretty consistent shorthand to indicate that we shouldn't go. It would have to be some pretty incredible content for me to rationalize seeing something with an "R" rating. One I can think of: Schindler's List.
