I agree with you that the technical was somewhat appropriate, given the the ref didn't know that he had blown such a huge call. So, yeah, the coach should be held accountable for that, but none of it would have happened if the ref had done his job correctly. Collins was held accountable, but the ref - who made a much stupider mistake - was never held accountable at all.
