A couple of outside the box thoughts

Could they get some sort of ground transport to a neighboring country (bus, train, car, etc)? Maybe they could get him to Italy, Germany, France, etc. Even buying a car and driving could be a relatively cheap option.

Could they fly in a Doctor? I'm sure that wouldn't be cheap but neither is a $150k flight home.

Tragic situation.
