I pay just north of $500 annually in premiums but then pay nothing for check-ups and cleanings twice a year which, for two adults and two kids, is basically is about the cost of the premiums. Pretty much everything after that is covered at 80% including the major stuff like crowns and root canals (everywhere else I've been covered that at 50%).
