Fan blade and metal fatigue possible cause of Southwest incident.
The engine is missing a fan blade and there was sign of metal fatigue at the site where the blade broke off.
It will likely be more than a year before the final NTSB report is released.
Southwest will accelerate the inspections of all engines of this type.
