I think the appropriate response is to write back first to the dentist
stating that you know who he is and where he works and you would like to know if that was a threat the dentist was making.

Let the dentist respond back and I would hope he would respond back and apologize for a completely inappropriate comment.

If the dentist doubles down then maybe some public shaming is in order. But I would give him the chance to apologize first.
