I try to be sympathetic due to it being a physical or mental condition.
Just like I try to be understanding of people in wheelchairs or with a walking cane.

I had a boss when I was young that cleared his throat every few minutes. He ended up with throat cancer in his early 40's. I had a consistent tickle in my throat and got worried. Turned out it was some kind of hernia in my esophagus that was treatable.
