In the case of most bigger employers they self-insure so the more people, dependent or employee, the more claims coming through to be paid out. To cover that they increase the amount they stick in the escrow account throughout the year.

Some employers will pass the extra cost of dependents to the employee but generally it is easier to administer by including them under the family plan without creating additional tiers.
