There is a one page application/certification that is very easy. I think it is probably the SBA form. It requires signatures of any over 20% owner, plus an officer certifying the application.

Our bank had an additional page to put the various calculations - wages, other payroll costs less disqualified wages, less disqualified benefits. One of the positives was that they allow us to use the 2019 Calendar Payroll records, so they require a copy of the year end, showing the total payroll, reconciled to the list of employees by salary, capping the over $100K wage employees and also excluding any benefits or payroll taxes for those folks.

We have four different payroll companies. The payroll register is easy, but then we have six different unions, so we needed monthly invoices for four of those and weekly invoices for two of those to send in, plus the 401K match invoices and health insurance invoices for our non-union company. Close to 200 pages of documentation. Submitted it all yesterday. Has to go through bank underwriting and I believe they have to certified they have verified it all.

I think we are just about wrapped up. They asked follow up questions today that took a couple of hours to put together, then later today they asked for a complete list of our shareholders and ownership percentages. I am hoping we are almost done and will receive word soon that we have been approved.

Hope this helps. Ours is a bit more complicated with four payroll companies, six unions, one non union company, etc. Looks like we will get $5.4 million. The interest rate is 1%, payback period is two years, no personal guarantees are required. It is a no-brainer, on the chance that any of it will be forgiven. I can't see any risk in doing it. I expect that we will get about 50% forgiven, which is good, we are going to need it.
