The form will ask if the individual was tested for coronavirus, the results of that test, and if they are experiencing symptoms. It will also require basic information such as a name and where the driver traveled prior to entering Utah.

These questions are great if you're trying to backtrace the start of an outbreak so you can inform populations that may have been exposed in order to prevent an outbreak becoming a pandemic, but it's a bit too late for that, isn't it?
