Sold our house in February and had to have the garage floor lifted.
The floor had settled, and was starting to put pressure on the foundation wall adjoining the garage. In order to avoid crushing that wall with the additional pressure of the polyurethane, they used a carbon fiber patch and some crazy good super glue over a couple of the cracks in the foundation. I think the patches added around $800 to the fee. Sounds like more work than what you have going on, and its not in Utah, but that's what I can offer.
